• Individual placement and support (IPS) investigated in moderate/severe mental illness.
• We document the long-term effects of IPS in Norway using an RCT and registry data.
• Effects were particularly large for younger individuals with low education.
• IPS proves to increase regular employment during a 43 months follow-up period.
• IPS is cost-effective compared to treatment-as-usual.
